This checklist walks you through exactly what information you’ll need, so you can apply for WAEPA’s exclusive coverage with confidence—knowing you’re fully prepared to complete your application in just a few minutes.

Federal Employment Information 

WAEPA proudly serves current and former Civilian Federal Employees. Because we’re a member organization built specifically for the Federal community, we need to confirm your Federal employment details to ensure eligibility. 

What you’ll need: 

  • Federal agency name (or last Federal agency) 
  • Federal employment start date 

Contact Information 

What you’ll need: 

  • Phone number
  • Email address 

Tobacco & Nicotine Use  

Tobacco use is a standard part of life insurance underwriting and helps determine your eligibility and premium rate. 

What you’ll need: 

  • Whether you currently use tobacco or nicotine products 

Your Desired Coverage Amount

Personal Information 

What you’ll need: 

  • Height 
  • Current weight 
  • Weight from 12 months ago 
  • Social Security number 
  • Marital status

Health Care Provider Information (Optional)  

While optional, providing your physician’s details can help speed up the review process if we need to confirm health information. 

What you’ll need: 

  • Physician’s first and last name 
  • Physician’s phone number 
  • Name of medical facility 
  • Facility street address 

Existing or Pending Life Insurance  

Knowing about other life insurance coverage helps underwriters better understand your overall coverage picture. 

What you’ll need: 

  • Details of any existing life insurance policies 
  • Information about any pending life insurance applications 

Beneficiary Information  

Beneficiaries are the individuals or entities who receive the policy benefit. Having this information ready helps ensure your wishes are carried out exactly as intended. 

What you’ll need: 

Individual Beneficiaries 

  • First and last name 
  • Relationship to you 
  • Percentage share of the benefit (must total 100%) 

Trust Beneficiary 

  • Name of the trust 
  • Date the trust was established 

Payment Information  

You won’t be billed unless you’re approved for coverage, but we collect payment preferences upfront to make activation seamless once your policy is issued. 

WAEPA accepts: 

  • Bank account or credit card for the initial premium 
  • Bank account information for ongoing premium payments

Medical Information 

Life insurance underwriting evaluates health history to determine eligibility and pricing. Being prepared helps avoid delays and follow-up requests. 

Personal Health History

You may be asked about: 

  • Current or past medical conditions 
  • Chronic illnesses 
  • Surgeries or major medical treatments 
  • Recent hospitalizations, including dates and locations 

Medications

Please have details for all medications, including prescriptions, over-the-counter drugs, and supplements: 

  • Medication name 
  • Dosage 
  • How often you take it 
  • Reason it was prescribed
  • When you first began taking it 
  • Any side effects or health impacts 

Mental Health History

You may be asked about: 

  • Treatment for conditions such as depression, anxiety, or bipolar disorder 
  • Current or past therapy or medications 

Height & Weight

These details help insurers evaluate overall health indicators such as body mass index (BMI). 

Family Health History 

Some medical conditions can be hereditary. Family health history helps insurers assess long-term risk factors. 

Be prepared to share: 

  • Medical conditions affecting immediate family members 
  • Additional context if there is a family history of certain illnesses
